NY State of Health has posted a training to help assistors learn how to submit account specific information on an encrypted spreadsheet. The macro enabled spreadsheet is called the Account Review Template.
Submitting cases in a standard format will allow assistors to streamline the account review process. Each assistor agency should designate a Reporter that reviews the cases prior to submitting them to the NYS Department of Health. The NYS Department of Health will then respond to your inquiry using the same Account Review Template. We are presenting this information so that assistors have a way to easily communicate with the NYS Department of Health.
