If you have been asked to provide documentation regarding your account, you can upload the documents in several ways:
- Log into your NY State of Health account at nystateofhealth.ny.gov.
- Meet with an enrollment assistor to receive assistance in uploading the documentation.
- Use NY State of Health’s new Mobile upload app through App Store® and Google Play™ to easily and securely submit documents right from your phone or tablet to NY State of Health.
