These links will bring you to the websites of the NY State of Health plans.  From there, you can search for your providers.  Plan provider networks can change from time to time.  Verify the participation of your provider when enrolling in a health plan.  If you need more information about a health plan's benefit or have specific questions about a plan's network, you can contact the health plan at the phone number provided.


The following provider network reports are based on information submitted by the Health Insurers participating in the Qualified Health Plan (QHP) and/or Essential Plan (EP) programs offered on the NY State of Health. The provider totals are unique totals, meaning that if a provider was submitted at multiple locations within the same county, that provider is counted once. These reports will be updated on a quarterly basis going forward. Consumers are encouraged to contact the provider(s) to confirm that they accept your health plan and participate in your network, before receiving health care services, or before enrolling in a health plan. In addition, if there are zero or a low number of providers available in your plan/county, you should contact your Health Insurer to assist with finding you a provider who is available in your network, or to discuss your options of seeing an out-of-network provider.
