    These double-sided cards give easy-to-read information about eligibility, benefits, and cost-sharing for Child Health Plus, the Essential Plan, and Qualified Health Plans. Please print, copy, link to and share.


    The $9 family premium contribution for children enrolled in the Child Health Plus program (households with income between 160% and 222% of the federal poverty level) has been eliminated for enrollment effective October 1, 2022.  The Child Health Plus At a Glance Card has been updated to reflect this change.

  • New York State Public Health Emergency Unwind Dashboard

    Under the Families First Coronavirus Response Act’s continuous coverage requirement, New York State Medicaid, Child Health Plus (CHPlus) and Essential Plan (EP) members have not had to renew their health insurance since early 2020. The Consolidated Appropriations Act of 2023 requires states to begin the process of redetermining enrollees in April 2023.
